PGA TOUR Superstore Leadership Program Application

The First Tee Leadership Series in partnership with PGA TOUR Superstore is an engaging and empowering five-week series designed to provide teens the opportunity to (1) build relationships with local PGA TOUR Superstore leadership, (2) uncover their leadership values and strengths, (3) identify potential careers based on those findings, and (4) develop goals on how to attain their desired career path. Facilitated by local PGA TOUR Superstore leadership, the five-week leadership series will be the first opportunity in a unique leadership pathway designed specifically for First Tee teens in store markets.

Because First Tee – Minnesota is part of PGA TOUR First Tee Foundation Network, quite a few Leadership Series participants from Minnesota have been chosen to participate in national programs offered by the First Tee Foundation.

Location: PGA TOUR Superstore, 12380 Wayzata Blvd, Minnetonka, MN 55305

Dates: Sundays, February 18 & 25, March 3 & 10 ( March 17 will be a makeup date)

Time : 4-6 pm

Ages: 14-18

Cost: Free

On March 13 there will be a town hall zoom call, not in-person (it is a National call for all participants).

Medical Emergency Treatment
In the event I cannot be reached in an emergency, I agree to accept any and all determinations of need for medical assistance and/or administration of medical attention deemed necessary by First Tee – Minnesota. I hereby give permission to the medical personnel selected by First Tee – Minnesota and/or PGA TOUR Superstore to secure any and all medical, hospitalization, dental, and/or surgical treatment. In event that such medical attention is needed from a healthcare provider, all costs shall be the responsibility of the parent or guardian.

Media Release
I hereby give my permission to First Tee of Minnesota and its authorized licensees to utilize, without compensation or further notice, the participant’s likeness, image, voice, name and/or their words incidental to any print, photographs, audio, video, television, radio, the Internet, social network or any other form or medium now known or hereafter devised for the purpose of promoting First Tee, their authorized licensees or for any other lawful purpose. The media will become the property of First Tee.

Assumption of Risk, Release and Hold Harmless Agreement
I, as the parent/guardian of the registering participant, give approval for his or her participation in First Tee and/or PGA TOUR Superstore sponsored activities. In full recognition of the dangers and hazards inherent in a golf and youth development program, I assume all risks of injury whatsoever and agree to release and hold harmless First Tee of Minnesota and other released parties from claim(s) of any nature arising from any activity, including transportation, connected with First Tee programs. This assumption of risk, release and hold harmless agreement includes, but is not limited to, any claim due to injury or loss proximately resulting from negligence of First Tee of Minnesota and/or PGA TOUR Superstore to the fullest extent provided by law.
